DOT Physicals

What are DOT physicals?

Federal law requires drivers of commercial motor vehicles (DMVs) to receive regular physical examinations. These exams are defined as the “Department of Transportation Medical Examinations.” DOT physicals are highly regulated for the drivers’ safety. They detect physical, mental, and emotional issues that can affect a driver’s ability to safely drive a commercial vehicle. DOT medical examiners are specially trained to understand the regulations and prevent drivers from being inappropriately disqualified.


Why did the FMCSA regulations change?

Driving a commercial motor vehicle means long, hard hours on the road. The Federal Motor Carrier Safety Administration (FMCSA) has updated the Department of Transportation (DOT) regulations to better reflect the physical demands drivers face and to help them be safer on the job. The new regulations require all medical examiners performing DOT physicals to:

  1. Be trained and certified in the FMCSA standards;
  2. Report exam results monthly to the FMCSA.

How can FAFP help?

Our DOT physicals are performed by board-certified, NRCME-certified clinicians who average 14 years of experience in medicine.  Dr. Johnson and Suzy Runkel are both certified in the FMCSA standards at our clinic.

There is a form on our website for anyone seeking a DOT physical.  Please print and answer the patient questions on the form and bring them to your visit.  DOT Physical Form

Unless we are contracted with your employer, you will be expected to pay for the visit at the time of service.  Patients will be provided with a receipt for reimbursement by their employer.  

Hearing Care

Comprehensive Hearing Care is now a fully-integrated service at France Avenue Family Physicians

 

Part of Every Complete Exam

Basic hearing assessment is now included for every patient at France Avenue Family Physicians. For many patients (age 55+), a simple hearing screening (30-60 seconds) is part of the normal workup before you see the doctor. For patients who need a more in-depth test, comprehensive exams are provided as an extension of your complete annual physical.

 

All of these services are included as part of the complete annual care we provide to our patients. No charge.

 

Though the program focuses on patients age 55+ (where hearing issues occur most frequently), it is open to every adult patient at France Avenue Family Physicians. If you are dealing with hearing problems yourself or suspect a problem with a family member, please tell anyone on our practice team. We will be happy to schedule a comprehensive exam.

Medicare Wellness Visits

Welcome to Medicare Insurance Coverage

In trying to assist you in understanding the complexity of Medicare insurance, we are providing you with some helpful information. 

As a newly enrolled Medicare subscriber, you have certain benefits available to you during the first 12 months you have Medicare.  This benefit is referred to as a Welcome to Medicare Visit.   

This visit is a preventive evaluation and management service.  The goals are health promotion and disease detection.  The components required in providing a “Welcome to Medicare Preventive Visit” are as follows;

  • Review of individual’s medical and social history.
  • Review of individual’s potential (risk factors) for depression and other mood disorders. 
  • Review of individual’s functional ability and level of safety.
  • A physical exam which includes only height, weight, blood pressure, BMI and visual acuity.  (This is not a hands-on exam.)
  • End of Life planning.
  • Evaluations/Referrals based on above. 

Please make sure that you inform the office when scheduling your appointment “this is my Welcome to Medicare Preventive Visit”, to ensure proper billing and reimbursement. 


Annual Wellness Visit

Medicare also offers another covered service for their patients (after 12 months of enrollment in Medicare), known as an “Annual Wellness Visit”.  This yearly visit is an evaluation for risk and determination of covered tests for Medicare, and is NOT a “hands-on” physical exam. 

The components required in providing an Annual Wellness Visit (AWV) are as follows:

  • Establishment of an individual’s medical/family history.
  • Establishment of a list of current providers and suppliers that are regularly involved in providing medical care to the individual.
  • Measurement of an individual’s height, weight, BMI (or waist circumference), and blood pressure. 
  • Detection of any cognitive impairment that the individual may have.
  • Review of the individual’s potential (risk factors) for depression based on the use of an appropriate screening instrument (PHQ-9).
  • Review of the individual’s functional ability and level of safety based on direct observation or the use of appropriate screening questions. 
  • Establishment of a screening schedule. 
  • Establishment of a list of risk factors and conditions for which primary, secondary, or tertiary interventions are recommended or are underway for the individual and a list of treatment options.

The Annual Wellness Visit DOES NOT require a hands-on physical examination to charge for the service.  Please make sure that you inform the office when scheduling your appointment “this is an Annual Wellness Visit NOT an Annual Exam, or Physical to ensure proper billing reimbursement. 

Lastly, Preventive exams (well physicals, hands-on exam) are NOT covered by Medicare.  These exams include a complete physical exam, immunization advice, screening tests (lab and x-ray) and guidance on topics such as diet and exercise.  Please make sure that you inform the office when scheduling this non-covered appointment “this is for my Annual Physical exam”.

DEXA Scans

A DEXA Scan uses a special X-Ray machine to measure your bone density. The DEXA Scan is a safe, easy screening test for Osteopenia and Osteoporosis that samples the density of bones at multiple sites. It can detect Osteoporosis before a fracture happens, can predict ones chances of fracturing bones in the future, and can monitor the effects of treatment of Osteoporosis.

 

Some risk factors of Osteoporosis include:

  • Small body frame (Weight < 127lbs or BMI < 20)
  • Low calcium Intake
  • Sedentary lifestyle
  • Smoking
  • Personal or family history of bone fracture
  • Estrogen or testosterone deficiency
  • Excessive Alcohol Consumption

 

The DEXA Scan is indicated for all post-menopausal women, and all men over the age of 70.

 

The DEXA Scan is a very simple and painless procedure that requires only a few minutes of your time. Routine views are taken of your hip and lower back. If you wear loose fitting clothing with no zippers, you will not need to undress for the procedure.

 

Preparation: Do not take calcium supplements the day of the procedure.

 

Following your DEXA scan, you will review your results with one of our providers.

Health maintenance exam

 

An adult health maintenance exam is a medical appointment that provides preventive services, counseling, education and disease screening, based on your age and gender.

A health maintenance exam has many names:

  • annual physical exam
  • preventive exam
  • complete physical
  • yearly checkup
  • wellness exam

What to expect

An adult health maintenance exam may involve some screening lab tests and/or shots. These services are usually part of an annual physical:

  • review of your complete medical history, including allergies, drug reactions and immunizations
  • physical exam, including a pelvic exam for women
  • medicine review (Please bring all medicines and supplements you regularly take.)
  • weight, height, blood pressure and pulse.

Your health care provider may ask about your tobacco and alcohol use. You may also discuss health topics like weight control and exercise.

Your health provider may recommend additional services based on your age and gender. An adult health maintenance exam may also involve lab tests, medical imaging and/or shots. Please check with your insurance provider about coverage for these extra services.

Extra Services

You may have other issues you want to discuss.  These services may not be considered part of your physical and may require an additional charge:

  • new health concerns (upset stomach, couch, warts, moles, etc.)
  • current conditions that need further evaluation, management and/or new treatment

 

Preventive services for adult women

The following are preventive services recommended by the American Academy of Family Physicians for adult women:

 

Ages 18 and up:

  • Accidental injury counseling
  • Screening and counseling for alcohol misuse
  • Screening for cervical cancer
  • Screening for chlamydia
  • Screening for congenital rubella syndrome
  • Counseling regarding benefits and risks of aspirin use to avoid coronary heart disease
  • Screening for depression
  • Screening for high blood pressure
  • Screening for obesity by measuring height and weight
  • Intensive counseling and behavioral interventions to promote sustained weight loss for obese adults
  • Counseling to maintain adequate calcium intake to prevent osteoporosis
  • Advice regarding physical activity
  • Counseling for parents who smoke regarding harmful effects of smoking on children's health
  • Counseling regarding the risks for STD's and how to prevent them
  • Screening for tobacco use and provide tobacco cessation interventions

 

Age 40 and up:

  • All of the above
  • Screening mammography every 1-2 years

 

Age 45 and up:

  • All of the above
  • Screening for high cholesterol

 

Age 50 and up:

  • All of the above
  • Screening for colorectal cancer

 

Age 60 and up:

  • All of the above
  • Screening for women at risk for fractures

 

Age 65 and up:

  • All of the above
  • Screening and counseling for hearing difficulties
  • Screening for osteoporosis
  • Screening for visual difficulties

 

Preventive screenings and exams are individualized for each patient according to their needs.  Additional screenings and counseling may be necessary according to risk factors.
